As a Community Reference Group member you accept that;

• your name may be released publicly as a representative on the CRG
• your participation in the CRG is a non-paid position
• you are available to attend scheduled meetings in person and constructively participate for the duration for the CRG?

If you are unable to meet these commitments and those stated within the full Terms of Reference, then you should not proceed with this expression of interest form.
